If you are a new patient or just needing to update your information, please fill out the above online forms. This will expedite everything if your paperwork is up-to-date when you come in. However, if you do not get around to it, do not stress about it. We can handle it when you arrive (you can also print out the forms below, fill them out prior to your appointment, and bring them in). What To Expect
Dr. Ashley Lloyd and her caring staff are dedicated to providing you with a pleasant visit. Please do not hesitate to call with any questions you may have about your upcoming appointment, insurance, or any other concern. We strive to make the process of receiving dentistry as simple as possible.
If this is your First Visit
Your first appointment will involve explaining your needs and treatment options . Dr. Ashley Lloyd and her team will get to know you, spending time discussing your needs, concerns, and desires so that together you may develop a treatment plan that will achieve an optimal result for you. A typical first visit involves a thorough exam, oral cancer screening, and cosmetic evaluation. Digital radiography(X-Rays) will typically be taken, and Dr. Lloyd will review them with you at this appointment. If you have not been to the dentist in a long time and you are worried about being berated about the condition of your mouth or teeth – do not worry. Our intent is to care for you regardless of whether you could afford care in the past. You will not be lectured or embarrassed about your dental care. We look at every person as the opportunity to do our best work.
Scheduling
We will schedule your appointment as promptly as possible and ask that you do the courtesy to let us know if you will be late or unable to keep your appointment. If you have pain or another kind of dental emergency, every attempt will be made to see you the same day you call.
We respect your time. If we expect to be delayed for more than 20 min. we try to call you ahead of time to allow you to reschedule or come a bit later so you will not have to wait. Emergency cases can and do arise and cause delays. We appreciate your understanding and patience.
